1) Validity Periods for Application Forms
| Form A: Community Partnership | Form B: Sustainability | |
| Validity Period for 2025/26 Applications | 1 January 2025 to 30 June 2026 (Any community partnership projects completed within this period and not previously used for applications in the 2024/25 cycle will be considered) |
Any consecutive 12-month between 1 January 2025 and 30 June 2026 |

| Important Notes on Submission | Please ensure all information provided is accurate, as no modifications can be made once the forms are submitted. | |
2) Application Forms Criteria
- Form A: Community Partnership
Each applicant can invite multiple nominators to complete Form A. The criteria from multiple forms will be combined and calculated; the combined criteria must meet a minimum of two (2) to fulfil the basic eligibility requirement for Form A.
- Form B: Sustainability
All data provided in Form B should fall within the selected reporting period.
